Job Description:
Carewest is a leading-edge public health care organization providing services for adults of all ages. Carewest offers a variety of Specialized Rehabilitation and Recovery Programs as well as Long Term Continuing Care. RESPONSIBILITIES
Responsible for assisting clients and families with social and financial issues related to continuing care admissions or return to the community. Collaborating with the Community Care Coordinator, Transition Services to assist with the development of the discharge plan. Communicating with the client and family about progress toward the appropriate discharge location.
Required Skills:
QUALIFICATIONS
Graduation from a recognized degree program in Social Work.
Registration with the Alberta Association of Social Workers; Driver's License
Minimum of two years experience in a combination of clinical and continuing care roles; Gerontology experience is preferred.
